OUR SERVICES - TELEPHONIC CARE MANAGEMENT

Telephonic care management provides a cost-effective method to manage the care of an injured or ill worker to assure that appropriate care is rendered with focus on returning to work as soon as possible. The injured worker, medical provider(s), employer, and care manager work as a team to assist the injured worker in the recovery and return to work process.

Seltzer Associates program will provide 24 hour, 365 days coverage. A Care Manager plans and monitors the medical care to assure appropriate, cost-effective treatment is provided for all injured workers. The Medical Care Manager will manage and monitor all medical care. If treatment is planned to be more extensive then 1 or 2 visits, or the injured worker is hospitalized, the nurse will consult with the treating provider(s) and develop a medical care plan. The medical care plan and a cost analysis will be presented to the adjuster for approval.

The function of the Care Manager includes, but is not limited to: review of referral information, contact with the injured worker, scheduling of physician appointments, facilitation of IME and second opinions, coordination of all medical services as appropriate by phone, and follow up with physician’s office to obtain work restrictions, release to work status, MMI and anticipated permanent impairment.
